24 Hour Cancellation Policy

If you miss your appointment or cancel your appointment without advanced notice, it may prevent Dr. Carter from helping other patients in need. In consideration to other patients and to our practice, kindly give 24 hours notice if you are unable to keep your appointment. Noncompliance with this policy will result in a late-cancellation fee.

Financial Policy

All fees, including co-payments and co-insurance payments, are due at the time of service. Payment is accepted in the form of cash or credit card. Checks are accepted up to $50.00.

Insurance Policy

Private Insurance: Dr. Carter welcomes most major PPO/POS plans. In order to provide the highest possible level of care, all other insurance plans are accepted on a case-by-case basis. This allows Dr. Carter to offer 1-hour consultations, minimal in-office wait time and 24-hour access via phone, email or pager. Please contact our office and we will be happy to verify your insurance coverage. If we are out of network with your insurance, we can help determine your out-of-network reimbursement benefits and come up with a mutually acceptable financial arrangement. We can also electronically submit your insurance claim on your behalf OR provide you with a completed claim form to submit at your convenience.

Medicare:	Sorry, the practice does not accept medicare at this time.
No-Fault:	No Fault patients must pay all fees upfront. The office will provide an itemized receipt to submit to your No Fault insurance for reimbursement. The office does not accept assignment on No Fault.
Medicaid:	Sorry, Dr. Carter does not accept Medicaid

Initial Visits

Your initial 1-hour visit will include an orthopedic examination. Your treatment records and diagnostic tests will be reviewed with you. You and Dr. Carter will create an individualized treatment plan based on your condition and your treatment goals. Dr. Carter runs her schedule on time. Please be on time for your appointment. If you arrive late, we will do our best to accommodate you, but we cannot guarantee your full appointment time. We encourage all new patients to register with our practice in advance of the initial visit using our new, secure online patient portal. Most therapeutic injections can be done on the same day as your visit.

Please Remember To Bring:

Any prescriptions given to you by your referring physician
Relevant imaging studies or diagnostic tests (X-rays, MRIs or CT scans); The actual images/CDs are preferable. However, if the images are unavailable then a report should suffice.
Prior medical records pertaining to your condition (including operative or procedure reports)
Your insurance card
Your completed patient registration and patient questionnaire forms
